Prior to your visit, please choose and submit the appropriate registration information:

• Completed Cosmetic Patient Registration (Click Here)

At the time of your appointment, please bring the following items:

• Current insurance card(s) and photo ID
• List of medications taken (prescription and over the counter)
• Referral (if your insurance carrier requires one)
• Old photographs clearly depicting upper and/or lower eyelids can be helpful to bring but are not required.

Our office accepts Medicare assignment and most major insurance plans. Please call our office with any questions regarding which specific insurance plans we accept. Insurance coverage and benefit questions should be directed to your insurance company. Consultations to discuss cosmetic procedures require payment at the time of your initial visit. If your visit is not considered cosmetic, we will gladly file your insurance for you. Co-payments and coinsurance are due at the time of your visit. We accept cash, VISA, Mastercard, and local checks with valid ID.
